Students who will earn more than 128 hours during the term of the scholarship will not be eligible for funds during the semester that the 12 hour minimum enrollment requirement exceeds 128 hours unless the Scholarship Committee has waived the requirement.

(List all college and community activities in which you are or have been involved and briefly describe the nature and extent of your participation in each activity. Include the year that you participated and the length of your involvement.)


List below the names of three references who are not your relatives. Do not obtain written recommendations.
