There are 4 tabs to choose from under this page. Each is for a different purpose. Please make sure to use the correct tab and web-form for your situation.
1) Accommodations Exams - If you have a student with an Accommodation through the Student Development Office, such as reduced distraction or extended time or some other accommodation, and you are not able to provide the accommodation, the Testing Center may be able to help. See this tab for details.
2) Online/Hybrid Proctored Exam Instructions - If you require a proctored exam as part of your online or hybrid course, please submit your exam instructions through this tab. Exam instructions should be completed a minimum of 2 weeks prior to the proctored exam start date, but preferably at the start of the semester. This ensures testing staff have adequate time to process and distribute exam instructions, and students can avoid higher proctoring fees for last-minute scheduling.
NOTE: This does NOT include exams that are set up as Respondus Record and Review.
3) Exam Extension Request - If you need to extend the available dates of a proctored exam for which you have already submitted exam instructions, you can use this form to extend exam dates for either a whole class, or selected students. Just remember to extend the dates in your actual exam in Blackboard/McGraw Hill/Pearson/etc.
4) Make-up Exam Submission Form - If a student misses an exam in any class because of Cameron University sponsored activity (such as participation in an athletic event) or due to a legal obligation (such as jury duty), the Testing Center will proctor the student's make - up exam. This service is not intended for students who miss exams for other reasons such as illness, childcare issues, etc. If your student meets these criteria, please use this form to send us the exam information.
